Job summary

Ascension in Jacksonville is hiring a Physical Therapist to lead clinical assessments and develop patient-centered care plans. You will deliver evidence-based interventions, mentor PTAs and students, and drive progress with data-driven evaluations to support timely discharge.

The role requires Florida-licensed PT credentials, BLS, and a commitment to high standards of care within a Catholic health system known for service and community impact.

Qualifications

• Bachelor's degree required; Master's or Doctorate preferred.
• Licensed Rehab Therapist (RPT) credentialed by Florida Board.
• BLS Provider within 30 days of hire; AHA or ARC accepted.
• No prior work experience required.

Your future role at a glance

• Lead clinical diagnostics by conducting comprehensive initial and ongoing assessments to evaluate mobility, strength, and functional needs.
• Develop patient-centered plans of care that prioritize individual goals and integrate family support for holistic recovery.
• Deliver evidence-based therapeutic interventions, ranging from manual therapy to neuromuscular re-education, tailored to each patient's unique pathology.
• Provide clinical leadership by mentoring and overseeing PTAs, students, and support staff to maintain high standards of care.
• Drive patient progress through data-driven evaluations, refining treatment strategies to ensure optimal outcomes and timely discharge.

How you’ll make an impact in this role

• Performs initial and on-going assessments of patient's condition.
• Establishes, revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family.
• Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
• Oversees activities of physical therapy assistants, students and other support personnel.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s): BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Licensed Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Florida Board of Physical Therapy Practice obtained prior to hire date or job transfer date.

Education

• Bachelor's degree required. Master's OR Doctorate degree preferred.
