Healthfirst is seeking a Pharmacy Technician MTM Navigator to review meds, address barriers to adherence, and coordinate treatment plans with pharmacists and partners. You will assist patients with refills, appointments, and access, ensuring comprehensive care.

The role emphasizes collaboration with community pharmacies and healthcare providers, collecting medication histories, and communicating solutions to maintain optimal therapy outcomes.

• Clear, concise, professional written and verbal communication experience.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Strong interpersonal skills for collaboration with health care teams.

Mansioni

• Review medication lists to support adherence and reduce barriers.
• Facilitate refills and new prescriptions; assist with appointments and access, documenting care.
• Coordinate MTM interventions with Healthfirst pharmacists and external partners.
• Monitor and communicate patient adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence and access to care.
• Handle member and provider communications via telephone or in person when needed.
• Perform other duties as required.
