Healthfirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing med lists and coordinating MTM to improve adherence and reduce gaps in therapy. You will work with pharmacists and partners to ensure effective medication plans and access for members in various settings.

Qualifications include a high school diploma or GED, and national certification or licensure as a Pharmacy Technician, with bilingual skills and MTM certificate as preferred enhancements.

Competenze

• High School Diploma or GED.
• Experience communicating with patients, providers and pharmacists.
• PTCB/NHA certification or state licensure as Pharmacy Technician.

Mansioni

• Review medication lists to promote adherence as the member's Pharmacy Navigator.
• Facilitate refills, new prescriptions, appointments, and access for financially challenged members.
• Coordinate MTM with Healthfirst pharmacists and partners within healthcare management services.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications by phone; in person when required.
• Perform additional duties as necessary.
