Job summary

Healthfirst is seeking a Pharmacy Technician MTM Navigator to optimize medication adherence and minimize gaps in therapy. You will review med lists, arrange refills and new prescriptions, and coordinate care with pharmacists and community pharmacies.

The role emphasizes communication with patients and providers, assessing barriers, and documenting care delivered. A national certification or state license is required, with bilingual skills a plus.

Qualifications

• High School Diploma or GED required.
• Experience communicating with patients, providers and pharmacists.
• National or state Pharmacy Technician certification in good standing.

Responsibilities

• Review medication lists to support adherence and reduce barriers.
• Obtain refills, set up new prescriptions, and coordinate access to meds.
• Communicate MTM plans with pharmacists and external partners.
• Monitor member medication adherence with community pharmacies.
• Collect patient med histories to identify adherence barriers.
• Handle member and provider communications by phone (in person when needed).
• Perform other duties as needed.
