Job summary

Healthfirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists, obtaining refills, and coordinating MTM interventions with our pharmacists and partners. The role includes assessing patient adherence, addressing financial barriers, and communicating with members and providers to optimize therapy.

The candidate will work with community pharmacies to monitor adherence and help ensure safe, effective medication management while complying with regulatory

Qualifications

High School Diploma or GED required.
Experience communicating with patients, providers, and pharmacists on medication issues.
Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/registered Pharmacy Technician.

Responsibilities

Review medication lists to ensure adherence and safety as the member's Pharmacy Navigator.
Obtain refills, new prescriptions, and appointments; assist with financial barriers to medications.
Coordinate MTM interventions with Healthfirst Pharmacists and partners.
Monitor member medication adherence with community pharmacies.
Conduct patient health assessments to identify barriers to adherence.
Handle member and provider communications by phone (in-person as needed).
Perform additional duties as needed.
