Job summary

Healthfirst is seeking a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists to remove barriers and improve adherence while supporting MTM interventions within the care team.

You will coordinate refills, assist with prescriptions, communicate with members and providers, and collaborate with community pharmacies to monitor adherence. Bilingual skills and MTM certification are preferred, and candidates must hold a national PT certification or state license.

Qualifications

• High School Diploma or GED completed.
• Experience communicating with patients, providers, and pharmacists.
• Nationally certified Pharmacy Technician (PTCB, NHA) or registered or licensed Pharmacy Technician in good standing.

Responsibilities

• Review medication lists to promote adherence and prevent adverse events.
• Facilitate refills, new prescriptions, appointments, and access to medications for financial barriers.
• Coordinate MTM interventions within broader healthcare management services.
• Collaborate with community pharmacies to monitor medication adherence.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications telephonically (in person may be required).
• Perform additional duties as necessary.
