Healthfirst seeks a Pharmacy Technician MTM Navigator to review medication lists, improve adherence, and coordinate MTM interventions with internal and external partners. You will help members obtain refills, set up prescriptions, and connect them with resources to overcome barriers to therapy.

The role emphasizes documenting care, communicating with providers, and supporting access to medications, including navigating financial assistance when needed. Bilingual skills are a plus.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating clearly and professionally with patients, providers, and pharmacists.
• Nationally certified Pharmacy Technician (PTCB/NHA) or registered/licensed Pharmacy Technician in good standing.
• Bilingual skills are a plus.

Responsabilités

• Review medication lists to remove barriers and support adherence as a Pharmacy Navigator.
• Assist members with refills, new prescriptions, setting up appointments, and financial access to medications.
• Coordinate and communicate a treatment plan or MTM intervention with Healthfirst pharmacists and partners.
• Monitor and communicate member medication adherence with community pharmacies.
• Perform patient health assessments of medication profiles to identify barriers to adherence.
• Handle member and provider communications by telephone; in-person visits as needed.
• Additional duties as necessary.
