Job summary

Healthfirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists to improve adherence and reduce gaps in therapy. The role focuses on enabling refills, new prescriptions, and coordination with providers to ensure safe, effective medication use.

You will work with Healthfirst pharmacists and external partners on MTM plans, communicate with members and providers, assess patient health data, and document care delivered.

Qualifications

• Strong written and verbal communication skills.
• Experience communicating with patients, providers, and pharmacists.

Responsibilities

• Review medication lists to support adherence and prevent adverse events.
• Facilitate refills and new prescriptions, set up appointments, assist with financial barriers.
• Coordinate MTM interventions with pharmacists and other partners.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications by phone; in-person when needed.
• Perform additional duties as assigned.
