Job summary

Healthfirst is hiring a Pharmacy Technician MTM Navigator to review medication lists, obtain refills, and coordinate treatment plans. You will work with pharmacists and external partners to ensure adherence and safe medication use, including communicating with members and providers.

The role requires strong communication skills, a pharmacy technician credential, and the ability to manage pathways for access to medications, including for patients with financial barriers.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating clearly and professionally in written and verbal form.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed Pharmacy Technician in good standing.

Responsibilities

• Review medication lists to improve adherence and reduce barriers to therapy.
• Arrange refills, new prescriptions, appointments, and access to medications for financial issues.
• Coordinate MTM interventions with Healthfirst pharmacists and partners.
• Monitor and communicate member medication adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence and access to care.
• Handle member and provider communications by phone or in person when required.
• Perform additional duties as needed.
