HealthFirst is seeking a Pharmacy Technician MTM Navigator to review medication lists, improve adherence, and coordinate MTM interventions for members. You will work with pharmacists and providers to ensure appropriate therapy and access to medications.

The role involves coordinating with community pharmacies, communicating with patients, and documenting care delivered. A high school diploma and valid pharmacy technician certification are required.

Competenze

• Strong written and verbal communication skills.
• Experience communicating with patients, providers, and pharmacists on meds.
• Nationally certified or licensed Pharmacy Technician in good standing.

Mansioni

• Review med lists to support adherence and prevent adverse events.
• Facilitate refills, new prescriptions, and appointments for members.
• Coordinate MTM interventions with pharmacists and providers.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments related to medications and access to care.
• Handle member and provider communications by phone or in person as needed.
• Perform additional duties as required.
