Job summary

HealthFirst is seeking a Pharmacy Technician MTM Navigator to review medication lists, remove barriers to therapy, and coordinate MTM interventions.

You will work with pharmacists and community pharmacies to ensure adherence and access to medications, including potential financial assistance, while documenting care and communicating with providers. A valid pharmacy technician certification or license is required.

Qualifications

• Minimum qualifications include high school diploma or GED.
• Experience communicating with patients, providers, and pharmacists.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed Pharmacy Technician.

Responsibilities

• Reviews medication lists to support adherence and identify barriers to therapy.
• Facilitates refills, new prescriptions, appointments, and access to medications.
• Collaborates with HealthFirst pharmacists and partners to coordinate MTM interventions.
• Monitors member adherence by coordinating with community pharmacies.
• Conducts patient health assessments to identify barriers to adherence.
• Handles member and provider communications via phone; in-person as needed.
• Performs additional duties as necessary.
