Job summary

HealthFirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists to promote adherence and minimize gaps in therapy. You will work with members, providers, and pharmacies to ensure timely refills and access to medications.

The role emphasizes MTM coordination with pharmacists and external partners, communicating treatment plans, and assisting with barriers to care, including financial challenges.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Experience with clear written and verbal communication.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/registered Pharmacy Technician in good standing.

Responsibilities

• Reviews medication lists to understand and eliminate barriers to therapy and acts as Pharmacy Navigator to promote adherence.
• Facilitates refills, new prescriptions, appointments, and access to medications, aligning refills to reduce pharmacy visits.
• Coordinates MTM interventions with HealthFirst Pharmacists and other partners within healthcare management services.
• Monitors and communicates member medication adherence with community pharmacies.
• Performs patient health assessments to identify barriers to adherence including patient factors and access to care.
• Handles member and provider communications telephonically (in person when required).
• Additional duties as necessary.
