Healthfirst is seeking a Pharmacy Technician MTM navigator to support medication therapy management and improve patient adherence across our care networks. You will review medication lists, coordinate refills and new prescriptions, and partner with pharmacists and providers to deliver integrated treatment plans.

Qualificações

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Experience with written and verbal communication in a professional setting.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/registered Pharmacy Technician in good standing.

Responsabilidades

• Review medication lists to understand and remove barriers to therapy and promote adherence.
• Assist members in obtaining refills, new prescriptions, appointments, and access to medications.
• Coordinate MTM interventions and communicate treatment plans with pharmacists and providers.
• Monitor member medication adherence by coordinating with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence and access to care.
• Handle member and provider communications telephonically or in person when required.
• Perform additional duties as necessary.
