Job summary

HealthFirst is seeking a Pharmacy Technician to act as a MTM-focused Pharmacy Navigator, reviewing medication lists, coordinating refills, and communicating care plans with providers. The role emphasizes adherence, reducing barriers, and collaboration across internal and external partners.

The ideal candidate will have a High School diploma or GED and national certification (PTCB/NHA) or valid licensure, with strong communication skills and bilingual abilities preferred.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed Pharmacy Technician in good standing.

Responsibilities

• Reviews medication lists to promote adherence and prevent adverse events.
• Facilitates refills, new prescriptions, and scheduling to reduce pharmacy visits.
• Coordinates MTM interventions with Healthfirst pharmacists and partners.
• Monitors and communicates member medication adherence with community pharmacies.
• Performs patient health assessments to identify barriers to adherence.
• Handles member and provider communications by phone (in person when needed).
• Performs additional duties as necessary.
