Job summary

HealthFirst seeks a Pharmacy Technician MTM Navigator to review medication lists, remove therapy barriers, and support adherence. You will arrange refills, new prescriptions, and appointments, working with pharmacists and providers to implement MTM plans.

You will assess patient medication profiles, communicate with patients and providers, and partner with community pharmacies to monitor adherence. Certification and state licensure in good standing are required.

Qualifications

• High School Diploma or GED required.
• Proven written and verbal communication with patients, providers and pharmacists.
• Current national or state pharmacy technician certification in good standing.

Responsibilities

• Serve as Pharmacy Navigator to improve adherence and reduce barriers.
• Facilitate refills, new prescriptions, appointments, and access for patients with financial issues.
• Coordinate MTM interventions within broader healthcare management services.
• Monitor and communicate patient adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence.
• Handle patient and provider communications by phone or in person.
• Perform additional duties as needed.
