Healthfirst in the United States is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists and coordinating MTM interventions to improve adherence and reduce gaps in therapy.

You will work with members, providers, and community pharmacies to obtain refills, coordinate new prescriptions, and document care delivered, while complying with licensing and certification requirements.

Competenze

• High School Diploma or GED is required.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB) or licensed Pharmacy Technician in good standing.

Mansioni

• Review medication lists to understand and remove barriers to adherence as Pharmacy Navigator.
• Facilitate refills and new prescriptions, set up appointments, and assist with financial access to medications.
• Coordinate MTM interventions with Healthfirst Pharmacists and other partners.
• Monitor member adherence by communicating with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications telephonically (in person where needed).
• Additional duties as necessary.
