Job summary

Healthfirst is seeking a Pharmacy Navigator to review medication lists, resolve barriers to therapy, and promote medication adherence for members.

You will coordinate MTM interventions with Healthfirst pharmacists and external partners, assist with refills and new prescriptions, and communicate with providers and patients to improve outcomes.

This role emphasizes clear documentation, collaboration with community pharmacies, and ensuring access to medications for patients in need.

Qualifications

• High School Diploma or GED is required.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed technician in good standing.
• Experience communicating with patients, providers, and pharmacists about medications.

Responsibilities

• Review medication lists to support adherence and eliminate barriers to therapy.
• Facilitate refills, new prescriptions, set up appointments, and assist with access to medications.
• Coordinate MTM interventions with Healthfirst pharmacists and partners.
• Monitor member medication adherence with community pharmacies.
• Perform patient health assessments to identify barriers to adherence.
• Handle member and provider communications by telephone or in person when needed.
• Additional duties as necessary.
