Job summary

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician in good standing or licensed.
• Preferred: bilingual skills and MTM certificate.

Responsibilities

• Review medication lists to optimize therapy and support adherence as Pharmacy Navigator.
• Facilitate refills, new prescriptions, appointments, and access for financially challenged members.
• Coordinate MTM interventions with Healthfirst pharmacists and partners.
• Monitor member adherence with community pharmacies and communicate with providers.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications telephonically; in-person as needed.
• Additional duties as necessary.
