Job summary

Healthfirst seeks a Pharmacy Technician to serve as a Pharmacy Navigator, coordinating MTM interventions and facilitating access to medications for members. You will review med lists, communicate with patients and providers, and collaborate with internal and external partners to support adherence and safe therapy.

The role involves documenting care, coordinating refills, and aiding in reducing gaps in therapy, with in-person duties possible in certain situations.

Qualifications

• Requires clear, professional written and verbal communication.
• Experience communicating with patients, providers, and pharmacists on medications.
• Licensed or nationally certified Pharmacy Technician in good standing.

Responsibilities

• Review medication lists to support adherence and reduce barriers.
• Facilitate refills and new prescriptions, setup appointments, and assist with access to meds for financial issues.
• Collaborate with Healthfirst pharmacists and partners to coordinate MTM interventions.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments to identify adherence barriers.
• Handle member and provider communications by phone (in-person as needed).
• Perform additional duties as necessary.
