Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant (PTA) to provide skilled care in patients' homes under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, and balance while ensuring safety and independence at home.

The role requires CAPTE-accredited PTA education, current licensure, CPR certification, and 1+ year of PTA experience in home health or related settings.

Qualifications

• Graduate from a CAPTE-accredited PTA program.
• Current state licensure to practice as a PTA.
• Current CPR certification.
• 1+ year PTA experience in home health or related setting.
• If <1 year experience, regional clinical leaders approval and mentoring required.
• Driver's license and reliable transportation.

Responsibilities

• Provide PTA services under supervision of a licensed PT.
• Document patient responses and changes to treatment plan.
• Support wellness and prevention; promote safety at home.
• Document supervisory activities in medical records.
• Monitor patient progress and report outcomes to the PT and team.
• Participate in care conferences and quality improvement activities.
• Assist with follow-up referrals and coordination with other care providers.
• Perform other duties as assigned.
