Job summary

UnitedHealth Group is seeking a Physical Therapist Assistant to deliver skilled PT care in patients' homes under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance, and independence while ensuring safety and quality of life at home.

Responsibilities include reporting patient progress, assisting with care coordination, and contributing to wellness and prevention efforts, within a supportive interdisciplinary

Qualifications

• Graduation from CAPTE-accredited PTA program.
• Current CPR certification.
• Current license to practice as a PTA in the applying state.
• 1+ year PTA/home health experience preferred.
• Driver's license and reliable transportation.

Responsibilities

• Provides physical therapy services under supervision of a licensed PT per care plan.
• Observes, records and reports patient responses to treatment and changes in condition.
• Provides resources for future needs and promotes wellness and prevention.
• Includes documentation of supervisory activities in the medical record.
• Monitors and documents patient responses to PT intervention.
• Participates in care conferences, in-services, chart audits and quality improvement activities.
• Maintains confidentiality of patient information and coordinates follow-up with other care providers.
• Performs other duties as assigned.
