Job summary

UnitedHealth Group is hiring a Per Diem Physical Therapist to deliver skilled PT services in patients' homes, focusing on mobility, function, and safety. You will collaborate with patients, caregivers, and an interdisciplinary team to support recovery and prevent hospitalizations.

Responsibilities include restoring movement, conducting periodic reexaminations, and planning discharges. A valid CAPTE-accredited education, state PT license, CPR, and at least 1 year of home health experience are

Qualifications

Graduate from an accredited physical therapy educational program as set by CAPTE
Current license to practice physical therapy specific to the state you're assigned to work in
Current CPR certification
1+ years of related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients
If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program
Driver's License and access to reliable transportation

Responsibilities

Helping patients maintain, restore and improve movement, activity and function
Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning
