UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant (PTA) to work under a licensed Physical Therapist, delivering skilled home health PT services and implementing individualized treatment plans to enhance mobility, strength, balance, and independence at patients’ homes.

You will collaborate with patients, caregivers, and the interdisciplinary team to support recovery, safety, and quality of life as part of a comprehensive care approach.

Qualifikationen

• Graduate from an accredited physical therapy assistant educational program as set by CAPTE.
• Current license to practice as a physical therapy assistant in the assigned state.
• Current CPR certification.
• 1+ years of related physical therapy experience for PTAs entering home health care to ensure knowledge and skills are sufficient to safely provide physical therapy services.
• If less than one year of experience, approval from regional clinical leaders and appropriate mentoring or residency program participation.

Aufgaben

• Provides physical therapy services and interventions under the direction of a physical therapist.
• Observes, records and reports patient responses to treatment and changes in condition.
• Provides resources for future needs and promotes wellness and prevention.
• Includes documentation of supervisory activities in the patient’s medical record.
• Monitors and documents patient responses to PT interventions.
• Participates in care conferences, in-services, chart audits and quality improvement activities.
• Maintains confidentiality of patient information.
• Makes follow-up care recommendations in coordination with the admitting agency.
