Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant to provide skilled PT services in patients' homes under supervision of a licensed PT. You will implement treatment plans to improve mobility and safety, and collaborate with patients, families, and the care team.

Requirements include graduation from a CAPTE-accredited PTA program, current PTA license, CPR certification, and 1+ year of PTA home health experience. A valid driver's license and reliable transportation are required.

Qualifications

• Graduate from a CAPTE-accredited PTA program.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ year of related PTA home health experience or mentorship/ residency if less than 1 year.
• Driver's license and reliable transportation.

Responsibilities

• Provide physical therapy services under supervision of a licensed PT per plan of care.
• Observe and report patient response to treatment to PT and clinical staff.
• Document supervisory activities and patient progress in records.
• Support safety, mobility, strength, and independence at home.
• Participate in care conferences, QA, and care coordination activities.
• Coordinate referrals and follow-up with agencies for continued care.
