Job summary

UnitedHealth Group is hiring a Physical Therapist Assistant to provide in-home skilled PT under a licensed PT. You will work with patients, caregivers, and the care team to improve mobility, balance, and safety at home.

Responsibilities include documenting progress, coordinating with providers, and participating in quality improvement activities. A CAPTE-accredited PTA program and state PTA license are required.

Qualifications

• Graduate from a CAPTE-accredited PTA program.
• Current license to practice as a PTA in the assigned state.
• Driver's license and reliable transportation.
• Current CPR certification.
• One year PTA experience in home health or acceptable mentorship.

Responsibilities

• Provide physical therapy services under supervision of a licensed PT per plan of care.
• Observe, record and report patient response to treatment to PT and clinicians.
• Provide resources for future needs and promote wellness at home.
• Document supervisory activities in the medical record.
• Monitor and document patient progress and adjust as needed.
• Participate in care conferences and quality improvement activities.
• Maintain confidentiality of patient information.
• Makes follow-up care recommendations and coordinates with other providers.
• Perform other duties as assigned.
