Job summary

Amedisys, part of the Optum family, offers Per Diem Physical Therapist Assistant roles delivering skilled care in patients’ homes. Under the supervision of a licensed PT, you’ll implement treatment plans to improve mobility and independence, while ensuring safety and quality of life at home.

Responsibilities include documenting progress, collaborating with care teams, and supporting transitions to follow-up care. A strong emphasis on confidentiality and compliance runs throughout the role.

Qualifications

• Graduate from CAPTE-accredited PTA program.
• Current PTA license for the state of assignment.
• Current CPR certification.
• 1+ years PTA home health care experience.
• If less than one year, regional leaders approval and mentoring/residency participation.

Responsibilities

• Provide PT services under supervision of a PT per plan of care.
• Observe, document, and report patient responses to treatment.
• Promote wellness and prevention, provide resources for future care.
• Document supervisory activities in patient records.
• Monitor patient response to PT intervention.
• Participate in care conferences, audits, QA activities, and peer reviews.
• Maintain confidentiality of patient information.
• Make referrals for follow-up care in coordination with the admitting agency.
• Perform other duties as assigned.
