Amedisys, part of the Optum family, is hiring a Per Diem Physical Therapy Assistant to provide skilled in-home therapy under a licensed PT. You will implement individualized treatment plans to improve mobility, strength, balance, and independence while promoting safety in the patient’s home.

You will collaborate with patients, caregivers, and an interdisciplinary team, documenting care and contributing to recovery and quality of life. Some visits may require driving and flexible hours.

Qualifikationen

• Graduate from an accredited physical therapy assistant program (CAPTE)
• Current license to practice as a PTA in the assigned state
• Current CPR certification
• 1+ years of PTA experience in home health care; less experience requires regional leadership approval

Aufgaben

• Provides physical therapy services under the supervision of a licensed physical therapist
• Observes and reports patient responses to treatment and changes in condition
• Provides resources for future needs and promotes wellness and prevention
• Documents supervisory activities in the patient’s medical record
• Monitors and documents response to therapy interventions
• Participates in patient care conferences, in-services, and quality improvement activities
• Maintains confidentiality of patient information
• Assists with referrals to community resources and follow-up care
