Amedisys, part of the Optum family, offers home health, hospice and community-based services. The Per Diem Physical Therapist will provide skilled PT services in patients' homes to promote mobility and independence, collaborating with an interdisciplinary team.

Responsibilities include maintaining and improving movement, conducting regular reexaminations, and coordinating discharge planning. A comprehensive benefits package is offered along with opportunities for professional development.

Qualifications

• Graduate from an CAPTE-accredited physical therapy program.
• Current license to practice PT in the assigned state.
• Current CPR certification.
• 1+ years of related PT experience for home health care.
• If less than one year, regional clinical leaders approval and mentoring.
• Driver's License and reliable transportation.

Responsabilités

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (at least every 30 days), modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
