Amedisys, part of the Optum family, is seeking a Per Diem Physical Therapist to provide skilled services in patients' homes, promoting mobility, independence, and safe functioning. You will collaborate with patients, caregivers and an interdisciplinary team to support recovery.

Responsibilities include improving movement, conducting reexaminations every 30 days, adjusting interventions, and discharge planning.

Qualificações

• Current license to practice physical therapy in the state you will work.
• Graduate from an accredited physical therapy educational program as set by CAPTE.
• 1+ year related physical therapy experience in home health care.
• If less than one year, regional clinical leaders' approval and mentoring/residency participation may be required.
• Current CPR certification.

Responsabilidades

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.
