Job summary

Amedisys, part of the Optum family, seeks a Per Diem Physical Therapist Assistant to deliver skilled care in patients’ homes. You’ll implement individualized plans to improve mobility, strength, balance and independence while promoting safety.

Under supervision of a licensed PT, you’ll collaborate with patients, caregivers and the care team, contribute to recovery, and support quality of life. Competitive per diem schedule and benefits are available.

Qualifications

• Graduate from an accredited physical therapy assistant educational program as set by CAPTE.
• Current license to practice as a physical therapy assistant in the assigned state.
• Driver's License and access to reliable transportation.
• Current CPR certification.
• One year related physical therapy experience for PTAs entering home health care, or approval from regional clinical leaders with mentoring.

Responsibilities

• Provide physical therapy services under the direction and supervision of a licensed physical therapist and according to the plan of care.
• Observe, record and report patient responses to treatment to the supervising PT and care team.
• Promote wellness, provide resources for future needs, and document supervisory activities.
• Document supervisory activities in the patient’s medical record.
• Monitor and document patient responses to interventions.
• Participate in care conferences, in-services, chart audits and quality improvement activities.
• Maintain confidentiality of patient information.
• Make follow-up care referrals in coordination with admitting agency and other providers.
• Perform other duties as assigned.
