Job summary

Amedisys, part of the Optum family, is hiring a Per Diem Physical Therapist Assistant to work under a licensed Physical Therapist in patients’ homes. You will implement individualized treatment plans to improve mobility, strength, balance, and independence, while promoting safety and quality of life in home settings.

Responsibilities include documenting patient progress, coordinating with caregivers and the interdisciplinary team, and supporting recovery through home-based care.

Qualifications

• Graduate from an accredited physical therapy assistant program as set by CAPTE.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ years of PTA experience for home health care or approval from regional clinical leaders if less than 1 year.
• Driver's License and reliable transportation.

Responsibilities

• Provides physical therapy services under the direction of a licensed physical therapist.
• Observes and reports patient response to treatment and changes in condition.
• Provides resources for future needs and promotes wellness.
• Documents supervisory activities in the medical record.
• Monitors patient response to therapy and progress.
• Participates in care conferences, in-services, and quality improvement activities.
• Maintains confidentiality of patient information.
• Makes follow-up care recommendations in coordination with care providers.
• Performs other duties as assigned.
