Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Topeka, KS and surrounding area. You will provide direct patient care, perform assessments, care planning, and education in home or alternate sites, coordinating with the care team.

The role requires an active RN license in KS/MO, BSN preferred, 3 years infusion experience, strong IV skills and port access, travel readiness, and proficiency with Outlook and nursing software.

Qualificações

• Current RN license in Kansas and Missouri in good standing.
• BSN preferred from an accredited program.
• Minimum of 3 years clinical nursing experience, infusion experience preferred.
• Demonstrated expertise in peripheral IV insertion and port access.
• Experience administering a range of infusion therapies and patient education.
• Willingness to travel within service area and maintain driving record.

Responsabilidades

• Provide direct patient care for assigned patients, including initiation and maintaining communications with the care team.
• Educate patients and care partners on safely administering home infusion therapy and provide additional instruction as needed.
• Assess home environment and identify interventions to ensure safety for prescribed care.
• Develop and evaluate the plan of care with the team and monitor response to therapy.
• Manage schedule and communicate changes to the team; coordinate patient care via approved platform.
• Participate in the nurse on-call rotation to provide continuous nursing support.
• Document all patient encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols and clinical standards.
• Ensure plans of treatment and clinical documents are submitted per policy and state guidelines.
