Job summary

Soleo Health is seeking a Per Diem Infusion Nurse in the Ottawa, KS area to support home infusion patients. You will assess, educate, and manage care plans for patients receiving infusion therapy, coordinating with the care team to ensure safe home care. Travel within the service area is required; KS/MO RN license and 3+ years nursing experience with infusion skills are required.

Qualifications

• Current RN license in Kansas and Missouri.
• BSN preferred from an accredited program.
• 3+ years of clinical nursing experience, infusion experience preferred.
• Experience with peripheral IV (PIV) insertion and port access.
• Experience with infusion therapies (biologics, IV hydration) preferred.
• CRNI/IgCN preferred but not required.
• ICU/CCU/Oncology/Vascular/Cardiac experience preferred.
• Strong communication and patient education skills.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Assess home environment and identify interventions for safe care.
• Develop and evaluate plan of care with team.
• Manage schedule via company platform and communicate changes.
• Coordinate patient care via approved platform.
• Participate in nurse on-call rotation.
• Document encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure timely submission of POTs and clinical documents.
