Job summary

Amedisys, part of the Optum family, is seeking a Per Diem Physical Therapist to deliver skilled services in patients' homes. You will help restore mobility, improve balance and safety while collaborating with patients, caregivers and an interdisciplinary team to support recovery and independence.

The role emphasizes flexible daily visits, adherence to CAPTE standards, and compliance with licensing, CPR and transportation requirements.

Qualifications

• Graduate from a CAPTE-accredited physical therapy program.
• Current licensure to practice physical therapy in the assigned state.
• Current CPR certification.
• 1+ year related PT experience in home health or willingness with mentoring.
• Driver's license and reliable transportation.

Responsibilities

• Provide skilled physical therapy services in patients' homes.
• Promote mobility, restore function, improve strength and balance, and enhance safety.
• Collaborate with patients, caregivers and an interdisciplinary care team.
• Conduct reexaminations every 30 days at minimum and perform discharge planning.
• Other duties as assigned.
