Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist to deliver skilled PT services in patients' homes, promoting mobility, function, and safety. You will work with an interdisciplinary team to support recovery and reduce hospitalizations, in a home-based setting.

Responsibilities include ongoing reexaminations every 30 days, adjusting treatment plans as needed, and discharge planning. A competitive benefits package is offered to eligible employees.

Qualifications

• Graduated from CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the assigned state.
• Current CPR certification.
• At least 1 year of related physical therapy experience in home health.
• If less than 1 year, approval from regional clinical leaders and mentoring or residency program.

Responsibilities

• Provide skilled PT services to patients in their homes to promote mobility and independence.
• Conduct regular reexaminations (at least every 30 days) and modify interventions as needed.
• Collaborate with patients, caregivers, and interdisciplinary team to support recovery and prevent hospitalizations.
