Job summary

Amedisys, part of the Optum family, seeks a Per Diem Physical Therapist to deliver skilled PT services in patients' homes to promote mobility, independence, and safety. You will collaborate with patients, caregivers and the care team to support recovery and quality of life.

Qualifications

• Graduate from CAPTE-accredited physical therapy education program.
• Current license to practice physical therapy in the assigned state.
• At least 1 year of home health PT experience; mentorship or residency if less.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations at least every 30 days, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
