Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• Graduate from a CAPTE-accredited PTA educational program.
• One year of PTA home health experience recommended to ensure safe PT services.
• If less than one year of experience, approval from regional clinical leaders and mentoring/residency participation may be required.
• Current CPR certification.
• Driver's license and access to reliable transportation.

Responsabilités

• Provides physical therapy services under the supervision of a licensed Physical Therapist.
• Observes and reports patient responses to treatment and changes in condition.
• Provides resources for future needs and promotes wellness.
• Documents supervisory activities in the medical record.
• Monitors and documents patient response to therapy.
• Participates in patient care conferences, in-services, chart audits, and quality improvement activities.
• Maintains confidentiality of patient information.
• Makes follow-up care recommendations with coordinating agencies.
