Job summary

Amedisys, part of the Optum family, is seeking a Per Diem Physical Therapist Assistant to deliver skilled care in patients' homes. You will work under a licensed Physical Therapist to implement individualized treatment plans that improve mobility, strength, balance and independence while promoting safety in the home setting.

Responsibilities include documenting outcomes, participating in care conferences, and coordinating follow-up with the team.

Qualifications

• Graduate from an accredited PTA program as set by CAPTE.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification is required.
• At least 1 year of PTA experience in home health or equivalent.
• Driver's license and reliable transportation for travel to patient homes.

Responsibilities

• Provide skilled physical therapy services under supervision of a licensed PT in patients' homes.
• Follow the physician-approved plan of care and document patient progress.
• Monitor patient response and report changes to supervising PT and care team.
• Coordinate care with caregivers and community resources as needed.
• Maintain accurate medical record documentation and confidentiality.
