Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist to deliver skilled services in patients' homes, promoting mobility, restoring function, and improving safety and independence. You will collaborate with patients, caregivers and an interdisciplinary team to support recovery and prevent hospitalizations.

Requirements include a CAPTE-accredited PT program, current state license, CPR, a valid driver’s license, and 1+ year of home health PT experience.

Qualifications

• Graduate from a CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the assigned state.
• Driver’s license and reliable transportation.
• Current CPR certification.
• 1+ years of home health PT experience; mentoring if less than 1 year.

Responsibilities

• Help patients maintain, restore and improve movement and function.
• Provide interventions, conduct reexaminations (min 30 days) and discharge planning.
• Collaborate with patients, caregivers and the interdisciplinary care team.
