Job summary

Amedisys, part of the Optum family, is hiring a Physical Therapist to provide skilled services in patients' homes, promoting mobility, balance, and independence.

You will work with patients, caregivers, and an interdisciplinary team to support recovery, prevent hospitalizations, and improve quality of life at home. A valid PT license, CAPTE-accredited degree, CPR, and home health experience are required.

Qualifications

• Graduate from a CAPTE-accredited physical therapy program.
• Current state PT license.
• Current CPR certification.
• 1+ years of home health PT experience or mentorship/Residency program.
• If less than one year, regional clinical leaders' approval required and participate in mentorship or residency.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Conduct reexaminations every 30 days and perform discharge planning.
• Collaborate with patients, caregivers, and an interdisciplinary care team.
• Other duties as assigned.
