OneWell Health Care is hiring a Per Diem Employment Specialist to support individuals with IDD in achieving competitive community-based employment.

You will conduct assessments, develop plans, and provide coaching, job search assistance, resume prep, and onboarding support across Pennsylvania. Field-based with local travel, you will coordinate with employers and interdisciplinary teams to promote independence and inclusion.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in human services or related field preferred.
• ACRE or CESP certification required within 6 months of employment.
• Experience with IDD or vocational services preferred.
• Proficiency in documentation systems and basic computer applications.
• Ability to use EHR or similar systems.
• Strong organizational and data tracking skills.
• Proficient in English (written and verbal).
• Valid driver’s license required.

Responsabilités

• Conduct vocational and career assessments.
• Develop individualized employment plans.
• Support individuals in securing and maintaining competitive employment.
• Assist with resume writing, interview prep, and workplace readiness training.
• Advocate for accommodations and promote inclusion.
• Maintain accurate service documentation and records.
