Job summary

Confluent Health is seeking a Care Coordinator to be the first voice patients hear and the welcoming face at the front desk. You will manage check-ins/outs, scheduling, insurance verification, and referrals with a patient-first approach.

Qualifications

• High School Diploma or GED.
• At least one year of customer-facing or patient-facing experience.
• Ability to manage multiple tasks while staying organized and detail-oriented.
• Comfortable communicating on the phone with patience and professionalism.
• Basic computer proficiency: scheduling software, emails, and spreadsheets.

Responsibilities

• Create a welcoming, professional first impression for patients, visitors, and referral sources.
• Handle front desk operations including patient check-in/check-out, scheduling, insurance verification/authorization, and phone calls.
• Review, schedule, and follow up on incoming referrals by gathering information and updating patient records.
• Keep patient flow and schedules moving by confirming appointments, managing waitlists, and following up for future visits.
• Collect copays, balances, and other payments accurately and professionally, with basic tracking or reconciliation.
• Protect patient privacy and uphold HIPAA standards in all interactions and documentation.
