Job summary

Confluent Health, LLC is seeking a Care Coordinator to warmly manage patient check-ins, scheduling, and payments, while coordinating with clinical teams to ensure a smooth patient journey.

You will handle referrals, insurance verification, and HIPAA-compliant documentation, with structured onboarding and ongoing support from clinic leadership. Training provided and opportunities for growth exist within the care network.

Qualifications

• High School Diploma or GED is required.
• Experience in a customer-facing or patient-facing role.
• Ability to manage multiple tasks while staying organized and detail-oriented.
• Comfortable communicating on the phone with patience and professionalism.
• Basic computer proficiency: scheduling software, emails, spreadsheets.

Responsibilities

• Create a welcoming, professional first impression for patients, visitors, and referral sources.
• Handle front desk operations including patient check-in/check-out, scheduling, insurance verification/authorization, and phone calls.
• Review, schedule, and follow up on referrals by gathering information and updating patient records.
• Maintain patient flow by confirming appointments, managing waitlists, and following up for future visits.
• Collect copays and payments accurately, with basic accounting when needed.
• Protect patient privacy and HIPAA standards in all interactions.
