Job summary

ChenMed is seeking a Care Facilitator to be a welcoming first contact for patients, handling check-in/out, scheduling, and basic administrative tasks. You will support patient experiences with accurate data entry, insurance verification, and courteous interactions, while coordinating with clinical staff to ensure smooth visits.

The role emphasizes exceptional customer service, strong communication, and reliability in a fast-paced ambulatory setting.

Qualifications

• High school diploma or equivalent education required.
• Graduation from a nationally accredited Medical Assistant program preferred.
• A minimum of 1 year of work experience in a medical clinic or similar environment required.
• BLS for Healthcare Providers preferred.

Responsibilities

• Check in/out patients and schedule appointments, greeting and assisting at the front desk.
• Serve as gatekeeper for PCP and Specialists’ schedules and ensure patient flow.
• Enter patient data in EMR, print forms and respond to emails promptly.
• Make love calls and send birthday cards to patients to strengthen relationships.
• Answer phones, relay messages accurately, and communicate clearly with staff.
• Verify insurance, collect copays, and schedule follow-up/ referrals as needed.
