Job summary

ChenMed is seeking a Care Facilitator to greet patients, check them in and out, and coordinate appointments in a fast-paced clinic environment. You will support scheduling, handle basic administrative tasks, and act as a patient advocate, delivering superior service with a positive, empathetic attitude.

The role requires strong communication, basic IT skills, and the ability to work with diverse teams. Evening/weekend availability may be needed as part of rotating shifts.

Qualifications

• Exceptional customer service mindset with a patient-first approach.
• Experience with medical insurance and EMR is preferred.
• Strong communication and data-entry accuracy are essential.

Responsibilities

• Greet patients, check-in/out, and scheduling appointments.
• Manage PCP and Specialist schedules and reminders.
• Enter patient data and forms into EMR; ensure accuracy of records.
