Job summary

ChenMed is seeking a Care Facilitator to be a welcoming first contact, handling check-in/out, scheduling, and basic clerical tasks in a primary care clinic. You will support patient flow, insurance verification, and follow-up appointments, while upholding ChenMed’s commitment to exceptional service.

The role requires high school diploma, Medical Assistant program preferred, and at least 1 year in a medical setting. Travel up to 10% may be needed in a regional scope. English fluency is essential.

Qualifications

• Exceptional customer service and positive attitude.
• Familiar with medical insurance and EMR systems.
• Proficient in MS Office (Excel, Word, Outlook).
• Willingness to travel locally up to 10%.
• Fluent in spoken and written English.

Responsibilities

• Greets patients, checks in/out and schedules appointments; maintains lobby organization.
• Acts as gatekeeper for PCP/Specialist schedules and ensures top patients are scheduled periodically.
• Uses computer systems to enter patient info, print forms, schedule appointments, respond to emails, and prepare new patient packets.
• Contacts and documents patient insurance info and collects co-pays; coordinates follow-up and referrals when needed.
• Guides patients to exam rooms and ensures comfort and safety.
• Communicates ChenMed clinicians to patients clearly and professionally.
• Escorts and supports patients during care processes and captures feedback via surveys.
• Participates in daily/weekly huddles and shares patient details.
• Manages transportation coordination, including Uber/Lyft for patients.
