Vital Living Healthcare is seeking a highly organized Patient Care Coordinator for our concierge medical and wellness practice in Atlanta. You will manage patient communications, scheduling, onboarding, and ongoing coordination with the care team to ensure a seamless patient experience.

Ideal candidates have at least two years of patient coordination or medical-office administration, strong notes, CRM/EMR experience, and a genuine commitment to attentive service.

Qualificações

• Minimum two years of patient coordination or medical-office administration experience.
• Strong communication, organization, and follow-through.
• Familiar with EMR systems and CRM platforms; able to manage multiple tasks.

Responsabilidades

• Serve as primary contact for patients via phone, text, email, and in-person.
• Schedule consultations, appointments, labs, and follow-up care.
• Coordinate referrals, records, and prescription-related requests.
• Maintain accurate patient, membership, and insurance information.
• Prepare and submit insurance claims and monitor status.
• Document activities in patient-management and CRM systems.
• Collaborate with care team and support daily operations.
