We are seeking a highly organized, service-oriented Patient Care Coordinator to join our concierge medical and wellness practice. Our services include personalized primary care, hormone replacement therapy, weight management, peptides, IV therapy, and other programs that help patients optimize their health and quality of life.
Our patients expect responsive communication, personalized attention, and thoughtful follow-through. In this role, you will help deliver that experience by connecting patients with the people, information, and services they need.

Additional support may include processing refill requests for practitioner review, communicating practitioner-approved information to pharmacies, and assisting with prior authorizations. All clinical duties must remain within the candidate's training, certification, and permitted scope. This position does not independently prescribe, authorize, or change medications.
Salary: $55,000 annually, plus a monthly team performance bonus based on shared practice goals and overall team performance.
