Job summary

ChenMed is seeking a Care Facilitator in the United States (Florida, Oakland Park) to be a welcoming patient advocate and first point of contact in our clinics. You will check patients in and out, schedule appointments, and provide basic secretarial support while delivering exceptional customer service.

The role requires handling insurance verification, copays, referrals, and confidentiality of patient information.

Qualifications

• High school diploma or equivalent required.
• Graduate from Medical Assistant program preferred.
• Minimum 1 year of work experience in a medical clinic or similar environment required.
• BLS for Healthcare Providers preferred.

Responsibilities

• Greet patients, check-in/out and schedule appointments; maintain lobby organization and cleanliness.
• Serve as gatekeeper for PCP/Specialist schedules and ensure key patients are scheduled every 13 days.
• Use computer systems to add patient information, prepare forms, respond to emails and generate patient packets.
• Make love calls, mail birthday and greeting cards to patients.
• Answer phones and take detailed messages for the appropriate staff.
• Communicate information about ChenMed staff to patients and others clearly.
• Verify insurance, collect copays, and schedule follow-up and referral appointments.
• Escort patients to exam rooms and ensure their comfort before leaving.
• Act as HEDIS champion for PCP and report relevant data.
• Present patient satisfaction surveys at checkout and escalate issues as needed.
• Participate in daily/weekly huddles to share patient details.
• Coordinate transportation, including driver lists and ride scheduling.
