ChenMed is seeking a Care Facilitator in Ohio to greet patients, manage check-in/out, schedule appointments and provide admin support. The role emphasizes exceptional customer service and patient advocacy within a fast-paced primary care setting.

The candidate will handle scheduling, data entry, communications with patients and staff, insurance verification, and follow-ups. Flexibility to work evenings, weekends, and holidays may be required.

Qualifications

• High school diploma or equivalent
• Graduation from a nationally accredited Medical Assistant program preferred
• A minimum of 1 year of work experience in a medical clinic or similar environment required
• BLS for Healthcare Providers preferred

Responsibilities

• Greet patients and check in/out, schedule appointments, and keep lobby organized
• Gatekeeper for PCP and Specialist schedules, ensuring regular patient scheduling
• Enter patient data, print forms, respond to emails, and update records
• Make love calls, send birthday and greeting cards to patients
• Answer phones, take messages, and route to appropriate staff
• Assist with insurance verification and copays; coordinate referrals
